There are six categories of Award this year and nominations can be made either by self-nomination or by being proposed by another person (proposer nomination).

1. Visit the Awards webpage and click on the relevant Award category document listed and complete the information required to support the nomination.
2. Complete the Nomination Form
3. Email the Nomination Form and supporting documents and information to Lee Shelley
Applications now close on Friday 30 April 2021.
